1. Using the acronym “A.C.T.I.O.N,” identify and explain the sequence the tool
recommends you follow to address all medication problems.
The A.C.T.I.O.N. tool helps PSWs safely deal with medication concerns while staying within
their role.
A – Assess the situation
The PSW observes the client and the medication. This includes noticing changes in the client’s
health, ability to take medication, refusals, or any differences between the medication label and
the care plan.
C – Consult the care plan and resources
The PSW checks the care plan, MAR, and agency policies to confirm what support is allowed
and what steps should be followed.
T – Talk with the client
The PSW speaks with the client to understand their concerns or difficulties. The client’s choices
and dignity must always be respected.
I – Inform the appropriate person
If the issue cannot be handled by the PSW, the nurse or supervisor must be informed right away.
O – Observe and support
The PSW continues to watch the client and provides safe support according to the care plan.
N – Note and document
All observations, actions taken, and people notified are recorded clearly in the client’s chart.
2. Situations
Situation 1
a. Main problem/challenge
The client has Parkinson’s disease and has trouble taking his medication on his own. His wife,
who normally helps him, is not home during the afternoon dose. This increases the risk of missed
or unsafe medication use.

Client observed struggling to self-administer oral medication due to Parkinson’s symptoms. Wife
unavailable during afternoon dose. Client unable to safely manage medication independently.
Supervisor notified for care plan review.
Situation 2
a. Main problem/challenge
The client’s arthritis is worse today, making it difficult for her to open the docette. She can take
her medication but needs help opening the container.

Assess / Observe and Support
c. Documentation-style action
Client reported increased joint pain and requested help opening docette. Docette opened as per
care plan. Client took medication independently. No issues noted.
Situation 3
a. Main problem/challenge
The client refuses to take his anti-inflammatory medication because he feels well and does not
think he needs it.
b. A.C.T.I.O.N. Plan section to use
Talk / Inform
c. Documentation-style action
Client reminded to take prescribed anti-inflammatory medication. Client refused, stating he feels
fine. Medication not taken. RN/supervisor informed. Client’s decision respected.
Situation 4
a. Main problem/challenge
The medication strength on the label does not match the care plan. This could be unsafe and
needs to be checked before assisting with the medication.

Assess / Inform
c. Documentation-style action
Medication label reviewed and strength differed from care plan. Client reported recent
medication change by clinic. Medication assistance withheld. RN/supervisor notified for
clarification.
