June 23, 2012 Dear Ms. Roces, Greetings from G&J Uni-Global Car Rental Inc.! Thank you for considering G&J and we value your business. We are pleased to extend our special rates for your transportation requirements. SELF DRIVE UNIT DAILY RATE TOYOTA INNOVA P 2,200.00 A/T DIESEL Transaction date: June 23, 2012 The above rates are quoted with the following terms and conditions: 1. 2. Rental Period: Daily- 24 hours. Insurance / Waiver: Rates includes Collision Damage Waiver (CDW), Personal Accident insurance (PAI), and Third Party Liability (TPL). Collision Damage Waiver (CDW) releases the renter from any liability in case of damage to the G&J vehicle in excess of the deductible franchise amount. The deductible franchise cost for Toyota Innova is P 9,000.00. The cost shall be borne by the renter in case of accident or property damage. Bodily Injury coverage is at P 1,000,000.00 per occurrence. Personal accident insurance (PAI) is P 50,000.00 per seat.

The compulsory Third Party Liability (TPL) insurance carried by all G&J cars cover the drivers liability for injury to other involved in accidents. 3. Fuel, Parking & Toll Fees: Each vehicle is supplied with full tank of fuel which is refilled at the renters expense upon return. Expenses for Fuel, Parking & Toll fees shall be for the account of the renter during the entire rental period 4. In the unlikely event of a breakdown, G&J should be notified for service or replacement. The vehicle shall not necessarily be of the same type as that of the original vehicle. 5. Payment: Cash rentals only, one hundred forty percent (140%) of the estimated rental charges will be required as prepayment. Refund, if any will be given after rental. 6. Validity of rate is fifteen (15) days from proposal date.
