SELF-MEASUREMENT FORM
Place the foot within the dotted outlines on the next page, hold the pencil UPRIGHT and draw an outline of
the sole of each foot. (If on carpet place a magazine under the sheet.) Lift the foot off the floor, and take the
measurements of each foot and leg OVER SOCKS and BREECHES at line shown in illustration.
Please note: If your feet do not fit within the length of standard printer size paper you can draw your outlines clearly, side by
side, on a blank piece of paper of any size. Please mark left and right next to the outlines.
