SAP Evaluation Form for Students

This form is used to determine if a student's status of failure to meet Satisfactory Academic Progress (SAP) standards is valid for the semester indicated. It requests information on whether the student has received an Academic Fresh Start or is pursuing a second degree. Evaluation results will be sent via email in about two weeks, and if denied the student must submit an SAP appeal by the posted deadline to receive financial aid for that semester.

2007-2008 EVALUATION FORM

This form should only be completed by students currently on SAP Failure, who have
been granted an Academic Fresh Start or who are pursuing a Second Degree. This form
is to determine whether or not your SAP Failure status is valid. This is NOT an SAP
Appeal.

after actual submission date.

DEADLINE for Submission:


Evaluation request must be submitted 2 weeks prior to start of classes to receive
financial aid for that semester (if eligible). Filing this form does not guarantee
reinstatement of financial aid eligibility. If evaluation request is denied, student must
submit an SAP Appeal during the semester for which financial aid is being requested
and before posted SAP Appeals date.
