Class of 2012 Apparel Order Form

This order form is for the Class of 2012 to purchase sweatshirts and shirts. Students can choose between sweatshirts for $25 or shirts for $5 in various sizes. There is an optional $6 charge to add personalized text on the back of a sweatshirt. Orders must be placed by lunch on February 8th by submitting the completed form along with a check payable to HHS ASB to the specified teachers' rooms. Pick up dates will be announced later. There is also an option to express interest in purchasing class sweatpants.
