PROBLEM BASE LEARNING

1st session on the 1st day


Medical History :
Mr.T, Caucasian 55 year-old engineer, was admitted to the ER after experiencing fever for a
week (>38oC). He had history of coughing, runny nose and subfebrile that occured 4 weeks ago
and he took some on-the-counter drugs and got better. But a week ago, he started to have an
excessive fatique and since 2 days ago he experienced shortness of breath and dizziness for a
week.
He lost his weight rapidly in a few months lately about 6 kg because he lost his appetite. No
history short of breath or abdominal pain; he is just fully stomach feeling. He denied history of
blood in stool or vomiting blood. He denied diarrhea.
He claimed bruises came up in his left leg and experienced gingival bleeding a few day ago. He
also mentioned that he got nose bleeding yesterday.
No trauma experiences before. No history of allergy, asthma, high blood pressure, diabetes and
heart problems. No pain when urinating. He never had tuberculin test and denied using tobacco,
ellicit drugs. He is married and only had sex with 3 female for his entire life including his wife.
He always had protected sex. He never had any surgeries or admitted to hospital before.
