Program Reward dan Punishment HSE
To control non-compliance with HSE requirements and procedures, the CONTRACTOR will issue a Non-Conformance
Report (NCR). Non- conformity means non-compliance with requirements that may deviate from relevant work standards,
practices, procedures, legal requirements or HSE requirements.
Each Non-Conformance Report (NCR) must be issued for any deviations and/or non-conformances from the
CONTRACTOR's management system including deviations from established HSE policies or standards, procedures, rules,
regulations and commitments.
9. DOKUMENTASI
9.1 10 GOLDEN RULE
10 Golden Rules didefinisikan untuk menetapkan aturan mutlak yang tidak dapat dilanggar karena menyebabkan
cedera serius atau kerugian bagi KONTRAKTOR dan PERUSAHAAN.
Fatality Prevention ini menjadi tanggung jawab yang harus dipahami untuk semua karyawan KONTRAKTOR dan
subkontraktor yang melakukan pekerjaan di lokasi kerja Proyek @@@@@@. Pelanggaran terhadap 10 Golden
Rules akan dikenakan punishment HSE dan peringatan kepada personil yang melanggar.
10 golden rule is defined to set absolute rules that cannot be violated for causing serious injury or loss to
CONTRACTOR and COMPANY.
This Fatality Prevention is a responsibility that must be understood for all CONTRACTOR and subcontractor
employees who carry out work at the @@@@@@@ Project work site. Violation of 10 Fatality Prevention will be
subject to HSE punishment and warning to personnel who violate it.
All Contractor employees entering the project area at the PT PP Presisi Tbk work site must receive Safety
Induction training before entering the project area. For new workers, an MCU medical examination and complete
personnel administration are carried out with the Contractor's HSER Representative.
After giving induction training and verification of personnel documents, the Contractor will issue an ID Badge.
When personnel are demobilized from the project, all ID Badges must be returned to the security gate personnel,
or HSE Department.
Before visitors arrive at the site, it is the host's responsibility to inform visitors of the minimum PPE requirements
for entering the area of the site that is relevant to their visit.
All Contractor employees must ensure all personnel attend approved safety inductions.
KONTRAKTOR memastikan bahwa Kebijakan Obat dan Alkohol berlaku untuk semua karyawan, subkontraktor,
atau pengunjung di lokasi operasional. Kontraktor berpedoman pada prinsip "zero tolerance" untuk
penyalahgunaan obat-obatan dan alkohol.
Konsumsi alkohol selama jam kerja dilarang sepenuhnya untuk semua aktifitas kerja yang dikendalikan oleh
Kontraktor. Semua karyawan diingatkan bahwa mereka dapat dikenai random check dan screening untuk
penyalahgunaan obat-obatan terlarang dan penyalahgunaan alkohol sewaktu-waktu selama mereka bekerja di
area kerja Kontraktor. Penyalahgunaan obat-obatan terlarang dan konsumsi minuman beralkohol akan
mendapatkan sanksi tegas.
Pemeriksaan awal dilakukan pada saat medical check up pre-employment dan pemeriksaan acak harian akan
dilakukan setiap hari oleh Site Paramedis untuk memastikan kondisi kesehatan pekerja dengan beberapa
kondisi sebagai berikut :
Pemeriksaan kesehatan bagi pekerja yang melakukan aktivitas berisiko tinggi.
Random check untuk pekerja di area kerja
Tes Narkoba dan Alkohol akan dilakukan secara acak. Namun, driver , operator dan karyawan dengan aktivitas
berisiko tinggi akan diprioritaskan untuk mengikuti tes.
Program ini dirancang untuk meminimalkan potensi seseorang melakukan pekerjaan di bawah pengaruh obat-
obatan dan/atau alkohol dan memastikan karyawan dan Kontraktor sadar akan bahaya melakukan pekerjaan
tersebut
CONTRACTOR ensures that the Drug and Alcohol Policy applies to all employees, subcontractors or visitors to
operational sites. The contractor is guided by the principle of "zero tolerance" for the abuse of drugs and alcohol.
Consumption of alcohol during working hours is strictly prohibited for all work activities controlled by the
Contractor. All employees are reminded that they may be subject to random checks and screening for drug
abuse and alcohol abuse at any time while they are working in the Contractor's work area. Abuse of illegal drugs
and consumption of alcoholic beverages will receive strict sanctions.
The initial examination is carried out during the pre-employment medical check-up and daily random checks will
be carried out every day by the Paramedic Site to ensure the health condition of workers with the following
conditions:
Health checks for workers who carry out high-risk activities.
Random check for workers in the work area
Drug and Alcohol tests will be conducted randomly. However, drivers, operators and employees with high-risk
activities will be prioritized to take the test.
This program is designed to minimize the potential for a person to perform work under the influence of drugs
and/or alcohol and to ensure that employees and Contractors are aware of the dangers of doing such work.
9.2.3 Night Work
Kontraktor harus memastikan bahwa penerangan yang disediakan cukup untuk pekerjaan malam. Kabel dan

beberapa mitigasi yang perlu diperhatikan antara lain namun tidak terbatas pada:
Rencanakan beban kerja yang sesuai dan bervariasi.
Tawarkan pilihan shift permanen atau rotasi dan cobalah untuk menghindari shift malam permanen.
Batasi shift hingga 12 jam termasuk lembur.
Dorong pekerja untuk beristirahat secara teratur dan berikan beberapa pilihan kapan mereka akan
diambil.
Pertimbangkan kebutuhan pekerja yang rentan, seperti pekerja muda atau lanjut usia.
Pertimbangkan untuk menyediakan minuman hangat & makanan dan zona makan yang tepat.
Kondisi cuaca akan menjadi pertimbangan untuk menyediakan pakaian tambahan agar tubuh tetap
hangat.
Pastikan suhu dan pencahayaan sesuai dan sebaiknya disesuaikan.
Informasi tentang risiko kerja malam hari dan memastikan pelaksanaan toolbox meeting dan
briefing.
Supervisor dan manajemen dapat mengenali potensi resiko dan masalah.
Pertimbangkan untuk meningkatkan pengawasan selama periode kewaspadaan rendah.
Melakukan pengawasan kesehatan untuk pekerja malam hari.
Penilaian pencahayaan (Pengukuran lux) akan selesai

The Contractor shall ensure that sufficient lighting is provided for night work. Cables and lighting devices must be
installed and maintained consistently with good electrical wiring and not on access points to prevent the cables
from being crushed by vehicles and heavy equipment.
The Contractor shall ensure that all gate points, pathways and assembly points are clearly lit and marked.
Stairway access and egress areas must be clearly lit.
The Contractor ensures that work area lighting is spaced properly to avoid glare and ensures adequate lighting of
the entire work area.
The Contractor ensures that the temporary lighting consists of non- conductive light sockets and their
connection is designed to be permanently attached to the insulation of the conductors. Light bulbs attached to
extension cords and light strings must be protected by a lamp guard.
The contractor made sure that no one was allowed to work alone at night.
HSE requirements are recommended before commencing work during night activities.
As with other health and safety hazards, the greater the exposure to shift work and extended working hours, the
greater the potential risk of harm. The risk of working long and continuous night shifts can potentially cause health
and safety problems. For this reason, there are several mitigations that need to be considered, including but not
limited to:
Plan appropriate and varied workloads.
Offer a choice of permanent or rotational shifts and try to avoid permanent night shifts.
Limit shifts to 12 hours including overtime.
Encourage workers to take regular breaks and provide options for when they will be taken.
Consider the needs of vulnerable workers, such as young or elderly workers.
Consider providing warm drinks & food and appropriate eating zones.
Weather conditions will be taken into consideration to provide additional clothing to keep the body
warm.
Make sure the temperature and lighting are suitable and preferably adjusted.
Information about the risks of working at night and ensuring the implementation of toolbox meetings and
briefings.
Supervisors and management can identify potential risks and problems.
Consider increasing surveillance during periods of low alertness.
Conduct health surveillance for night workers.
Lighting assessment (Measurement of lux) will be completed according to Regulation no.5/2018 Make
sure the workplace and surroundings are okay, safe and secure.
Minimum requirements must be identified as well as control measures to minimize the occurrence of Heat Stress,
identifying symptoms associated with Heat Stress and actions to be taken if Heat Stress has been identified,
including but not limited to:
The contractor must ensure that an adequate supply of drinking water, provide worker with disposable
cup /glass or personal water container (tumbler) for drink, together with shaded rest areas, is provided
around the work area. provided easy access to water and rest area to prevent Heat Stress.
All employees who are exposed or potentially exposed to Heat Stress will receive training on injuries
and precautions related to Heat Stress.
The contractor must ensure that all employees understand the early signs of heat stress.
9.2.6 Lone or Isolated Work

9.2.7 Drilling Work

Periksa apakah semua sumber daya seperti mesin, peralatan, dan bahan disimpan, dipesan, dan ditandai
dengan benar.
Periksa bahwa jalur, rute pelarian, peralatan pemadam kebakaran, dan fasilitas darurat lainnya ditandai
dengan benar dan tidak terhalang.
Dalam kegiatan jarak jauh, sarana komunikasi yang aman harus ditetapkan (telepon radio atau satelit)
Pastikan tempat kerja hanya berisi alat, suku cadang dan material yang akan digunakan untuk melakukan
kegiatan pengeboran.

secara mekanis atau manual dari penutup tanah, perkerasan atau lantai di dekat atau di bawah pengaruh
saluran, pipa, galeri, terowongan atau jaringan energi bawah tanah diadopsi minimal, sebagai berikut
kriteria:
- Evaluasi rinci proyek eksekutif dan/atau konstruksi dengan spesifikasi gangguan (menggunakan
georadar).
- Jika berlaku dan perlu, mintalah gambar "as built" untuk menemukan gangguan bawah tanah.
- Pelaksanaan survei manual (bila ada).
Jika berlaku, pelepasan resmi dari area di mana pengeboran, penggalian, atau layanan gangguan
lainnya akan dilakukan harus diminta.
Simpan semua dokumen yang diperlukan di depan pekerjaan, seperti: ART, PTS dan daftar periksa
peralatan, bukti pelatihan (lencana/paspor, dll.) dan lain-lain yang relevan dengan jenis kegiatan ini.
Pasang bak penampung di sebelah peralatan pengeboran yang digerakkan oleh mesin diesel, yang
komponennya digunakan dengan minyak pelumas dan gemuk, yang rentan bocor selama

All excavation operations must be carried out with due regard for:
Prior to commencing excavation, inspectors should be informed of the proposed work to locate all
underground, sewer, telephone, water, electricity and gas installations. Overhead hazards must be noticed
and dealt with directly.
Procedures for obtaining work permits under certain conditions.
Supervisors and HSE personnel should inspect excavations and trenches after rain and when soil
conditions change.
If there is stagnant water in the excavation area and it can endanger workers, then the water pump is
installed and operated by a competent person.
If the stability of a building or wall is threatened with landslides as a result of digging or ditches, then the
excavation is given protection to secure the work.
Protection should be provided by placing fences at least 1 meter from the edge of the excavation, or by
clearing the excavation edge of material to prevent material or equipment from falling or rolling over into
the excavation area.
Access in the form of stairs with handrails must be provided as access to go up and down the
excavation area.
Physical barrier (handrail or any hard barrier) using safety tape is not allowed.
A safe work method statement will submit along with complete drawing, ground stability check, cross
validation with Geo-technical survey.
If area is left unguarded, a slope stability check shall be done by
qualified personnel before resume the activit.
Every work at height must have a safe work permit including an emergency rescue plan and work safety
analysis.
Fall protection devices must be fitted for any activity that has the potential to fall from a height.
Working 1.8m must use a safety full body harness complete with a double lanyard.
Shock absorbers will be required to be fitted to the full body harness taking into account the potential
fall distance (with a safety factor of 3 feet). Below is an illustration of the image for calculating the potential
fall distance.
Workers who work above 10 m must be checked by the site paramedic to ensure worker fitness
(blood pressure, roomberg test & others) before starting work.
Work at height will not be permitted without supervision from an authorized person.
If a permit is required to work, it must be issued under the authorization of the Construction Manager.
Verification of work completion will be carried out by the Project HSE Team
o Installation, modification and dismantling of scaffolding must be carried out by a competent person (as
usual, in a team of at least one person as a certified scaffold) and inspected by a certified scaffold
inspector (DEPNAKER certificate).
o Employees must not access scaffolding that has not been properly inspected and tagged.
o Work platforms must be provided with standard guardrails, center rails and footboards.
Contractors and Subcontractors must implement several safety procedures to minimize the risk of
slipping, tripping and falling by:
• Housekeeping must be maintained properly to limit or prevent slips, trips and falls.
• When working at heights, care must be taken to move and climb safely, taking into account slippery
surfaces such as steps.
• Adequate lighting should be provided for areas where there is a potential for slipping, tripping and falling.
• Hazards/obstacles that can cause slips, trips and falls must be clearly identified and marked.
• To maintain non-slip surface conditions and use non-slip surface tools.
• Practice good cable management to minimize the potential for slips, trips and falls.
• Work platforms in elevated areas must be equipped with handrails, such as scaffolding platforms, ladders,
pipe rack platforms.
• Anti-slip footwear must be provided for all Contractor and Subcontractor workers.
The most effective way to eliminate/minimize potential exposure to personnel from falling tools, materials and/or
debris is to never allow employees to work and walk under areas where there is work at height.
to secure the lower area above which there is elevation work by installing barricades.
Tools, equipment, materials, etc., will be stored and managed in an orderly manner and with good
housekeeping.
Controlling/limiting the amount of material brought into the work area at height.
Minimize hand-to-hand carrying or handing of materials.
Use, baskets, containers, etc. for lifting tools and materials.
Tie scaffolding pipes together, installed with a minimum of two chokers/slings and will be kept
horizontal.
Materials must be properly secured to prevent the influence of wind.
Scaffolding and work platforms must be provided with footboards at the edges.
Tools and equipment will be stored in suitable containers/boxes to prevent objects from falling.
Hot work will not be carried out above without being protected by a fire blanket to prevent potential hot
iron dregs/melting, sparks, or molten metal falling down.
Tools, materials, will never be placed or stored on beams, pipes, valves unless they are secured.
Appropriate cover tools must be used when working at height
9.2.15 Persyaratan crane dan liftting gear / Crane requirements and lifting gear
Persyaratan crane
Kontraktor akan memastikan bahwa semua crane yang digunakan untuk proyek ini baik yang disediakan
oleh Subkontraktor dan/atau langsung oleh Kontraktor harus memiliki sertifikat yang sah dari DEPNAKER.
Semua crane harus memenuhi standar Kontraktor dan hanya dapat digunakan setelah mendapat
persetujuan dari Kontraktor.

Rencana pengangkatan

pengangkatan (Lifting Plan) dan metode serta menyerahkannya kepada Koordinator HSE Kontraktor dan
Construction Manager untuk ditinjau. Pernyataan rencana dan metode pengangkatan akan dilampirkan di
permit to work. Rencana pengangkatan ini akan disampaikan paling lambat dua hari sebelum pelaksanaan
pekerjaan.
Lifting Plan dan Izin Kerja khusus harus dibuat untuk semua kegiatan terutama untuk kondisi berikut:
- Ketika pengangkatan dengan beban 80% dari SWL crane.
- Bentuk tidak beraturan misalnya beton, plat, pipa, batang, dll.
- Bahan berharga (lihat biaya tingkat risiko tinggi)
- Bahan berbahaya
- Cuaca (kecepatan angin maksimum 20 knot)
- Ketika pengangkatan yang melintasi area plan proses eksisting.
- Untuk pengangkatan dengan menggunakan crane diatas 80 ton
- Ketika operator crane tidak memiliki pengalaman untuk mengangkat beban tertentu
- Untuk pengangkatan yang kompleks, JSA harus dianalisis secara menyeluruh (misalnya crane
Crawler melakukan pengangkatan tandem bergerak dengan beban)
- Karena adanya bahaya kerja malam hari, kegiatan ini mungkin diperbolehkan untuk dilakukan

To control traffic on the project, the contractor will develop a traffic management plan to prevent traffic accidents
from occurring in the workplace. In preparing the traffic management plan, the post office will carry out an
analysis and determine the following items:
Initial risk assessment of traffic access conditions in the project area
This initial traffic risk assessment was carried out to determine the traffic risks in the project area. The
contractor will collect risk data resulting from a risk assessment which will then be reviewed to determine
the appropriate traffic risk control.

- Analyze traffic density and the types of vehicles and mobile equipment that will pass through the
project road.

and drainage systems.
- Define standard roads to be built and used.
- Road inspections must be carried out periodically to ensure that the road conditions are in good
condition for use and to ensure that the maintenance of the road program is carried out periodically.
All driving on mining roads must comply with posted traffic signs.
All Light Vehicles, Equipment, Buses, Trailers must pass inspection by the Contractor.
Drivers of all vehicles must have a driver's license from POLRI and Company SIMPER.
All loads must be properly secured, all passengers must fasten their seat belts before moving.
For the transportation of material or equipment that is too large, it must be escorted by a security officer.
Workers are not allowed to ride behind the pickup, but it is also prohibited to carry workers and materials
together.
Carry out daily physical checks regarding the roadworthiness of their vehicles prior to use.
Not driving when sick or taking any medication that could affect driving judgment.
Wear safety shoes and safety helmet when exiting their vehicle/equipment in the construction site area.
Ensure that there are no oil leaks from vehicles/equipment that enter the project site which can
contaminate the soil and consequently the groundwater system.
To park only in designated parking lots.
To report all accidents and incidents.
The use of compressed gas cylinders will follow the standards of the Ministry of Manpower (Ministry of Manpower
Regulation No. 37 of 2016 concerning Pressure Vessels and Storage Tanks) in terms of: how to store, how to
handle, how to transport and use. Compressed gas cylinders are stored using racks and tied using chains.
The contractor must ensure that all gas cylinders are labeled to indicate the contents of the cylinders.
Flammable gas and oxygen cylinders must be equipped with a flashback arrestor on the side of the torch
and regulator.
The contractor must prepare a gas cylinder inspection schedule.
All compressed cylinders must be kept away from oil, grease or other hydrocarbons.
Cylinders should be checked for leaks and/or defects before use.
Cylinders must not be exposed to sparks, slag, flames, or other sources of heat.
Cylinders must be secured in an upright position during use and storage. They must be secured using
Chains, (never use tape or rope barricades).
A protective cap must be fitted over the valve stem when the cylinder is stored or not in use.
Cylinders must not be brought into confined spaces.
When cylinders are not in use, cylinders of oxygen and fuel gas must be stored separately.
Contractor provides cylindrical carts (portable racks) with wheels and easy to transport.
Requirements for flash back arrestors and No return (check) valves to be made in accordance with MHS12.
The storage area for cylinders must be marked with a sign stating, “Full or Empty Oxygen,” Full or Empty
Acetylene, etc.

The possibility of fire/change in pressure or gas is not more than 2%
Must not contain toxic substances such as H2S, H2SO4, Cyanide, and others.
Any atmospheric condition hazardous to life and health
If hot work is carried out in a confined space, the HSER requirements on the conditions inside and the equipment
used must comply with the "Hazardous Work Permit Procedure", when an employee enters the confined space
containing flammable or toxic substances, a lifeline must be equipped. CONTRACTOR management and
subcontractors shall be responsible for ensuring that confined space procedures are carried out in accordance
with COMPANY HSER procedures and regulations.
Contractors to identify a list of equipment or area workers who have the potential to work in confined spaces.
Identification of the area is given a special identity.
A risk assessment should be carried out for work in the designated workspace.
The list of supporting equipment for work in confined spaces is as follows:
- Gas tester
- Blowers
- Lighting
Every workforce that will work in confined spaces must have attended training to work in confined spaces.
Requirements for PPE and safety equipment for working in a confined space that must be prepared are:
Life support equipment with a backup power source (battery)
which can automatically be used if there is a power failure.
The supply of air cylinders is arranged in such a way that they are free and provided for primary and
secondary air supply.
Breathing apparatus for emergency (10 minutes capacity).
Tool for monitoring oxygen content, LEL and temperature to monitor conditions in confined spaces.
Temperature gauge to check temperature in order to monitor conditions in confined spaces.
Well-functioning communication tools, so that the staff on standby, workers working in confined spaces
and operators of life support equipment function properly when working in a stack. Communication tools
with workers in the stack work automatically without having to press the talk button. The communication
equipment used must be integrated with the air supply equipment, the equipment must have explosion
proof.
Lighting used in confined spaces must be categorized as explosion proof and not exceed 50 V.
Full body harnesses.
Mechanical equipment such as a winch that is capable of preventing workers from falling and can be
used for evacuation in an emergency.
Restricted areas around open manways must be equipped with continuous O2 monitoring devices in
atmospheric air.
Resuscitation oxygen cylinders for artificial respiration around the area are provided by Pertamina and
officers who are competent in performing artificial respiration (CPR).
Safe working time in a confined area depends on the high temperature to which it is exposed. The table below is
used to show the length of time worked for workers working in confined space conditions:
All accidental fires on the Contractor's property, however small, must be reported immediately to the HSE
Coordinator. The official report should include all known or reasonable summary details as additional reports may
be required by government agencies.
Fire prevention is very important. Good housekeeping and equipment maintenance should be followed to
minimize fire hazard.
Contractors and Subcontractors must provide a gas detector if working in a hazardous area, the gas detector
used will be calibrated annually. Contractor will conduct Fire Protection Training to employees. Storage and use
of flammable and flammable liquids will comply with NFPA 30. All fire protection must be collected in
designated areas which are clearly identified with appropriate markings. This equipment must be located
near potential fire hazards but must also be accessible to operating personnel. The number of types and
locations o f fire extinguishers must comply with applicable standards.
9.2.20 Penyimpanan & penanganan bahan yang mudah terbakar / meledak / Storage & handling of flammable /
explosive materials
Berikut ini adalah persyaratan untuk penyimpanan bahan yang mudah terbakar / meledak:

The following are requirements for the storage of flammable/explosive materials:
• Because evaporation (formation of vapour) increases when the temperature is increased or when
more exposure to air this liquid occurs, it is very important to store it in a relatively cool place and in a
closed container. Open containers should not be used to store flammable and explosive materials.
• Storage in open areas - in field conditions drums should be placed so that they are not exposed to direct
sunlight, or other sources of heat.
• The storage area must have a clearly displayed warning sign indicating that flammable and explosive
materials are stored there, and no smoking is permitted and an explosion proof lamp shall be mandatory.
• Shallow ditches or dikes should be constructed around storage areas to prevent spillage or spread of
vapors to lower ground areas where they can contact fire sources and start fires.
• Storage areas should be at least 50 feet from sources of ignition (eg open flames, workshop areas where
grinding tools or flame cutting equipment are used).
• Indoor storage - ideally, storage of flammable and explosive materials should be in a separate isolated
building. If this is not possible, they should be stored in special rooms with fire-resistant walls, doors,
ceilings and floors.
• Indoor storage must be protected by an automatic fire protection system (sprinkler, foam and carbon
dioxide).
During the transfer of flammable liquids from one container to another, there is a possibility of static electricity
being generated which is a dangerous source of ignition. To prevent the build-up of static electricity, the
following requirements are required during the handling of flammable liquids:
• Storage of flammable liquids must be equipped with earthing facilities. This is to minimize the potential
for static build-up during the transfer of flammable liquids.
• Grounding facilities for storage must be tested periodically to ensure they are still functioning properly.
• Safety glasses, protective gloves and long sleeve uniform should be worn if there is any possibility of
skin contact. Liquid should be washed off the skin with non-abrasive soap or hand sanitizer. It is
recommended to provide safety shower facilities and/or an adequate water supply near storage areas.
• Liquids should be used only in well-ventilated areas to avoid inhalation of vapors and fumes. If
ventilation is not possible ([Link] a confined space), an approved respirator must be worn.
If any personnel are splashed with hazardous liquid or caught fire, they must be taken immediately to a
source of running water and the splashed area must be doused with water for at least 15 minutes.
Contaminated clothing should be removed, and immediate medical attention sought.
Welding cutting and grinding or work that generates flames or other sparks are activities that are included in hot
work. All hot work activities must be carried out with a hot work permit and job safety analysis.
Hot work areas must be safe from flammable or explosive gases. Gas test must be carried out prior to work and
continue to be monitored until the work is completed.
Areas within 10-11 meters of hot work shall be free of flammable materials such as dry wood, trash, fuel and
chemicals.
All personnel performing cutting, welding and milling, including welders, fitters and helpers must be trained
The Contractor will ensure that all substances classified as hazardous to health, which are transported, used or
created during the activity, are controlled to minimize risks to health and the environment.
• List of Chemicals and Hazardous Substances to be used must be available including the Safety Data
Sheet (SDS).
• The required PPE as stated in the SDS must be worn during handling with chemicals.
• Necessary equipment and tools as specified in SDS to control accidental release of hazardous
chemicals must be provided.
• Contractors may not bring Asbestos or materials containing Asbestos to the Site.
• Prior to starting a task where hazardous materials will be used, the risks, precautions to be taken and
actions to be taken in the event of a spill (if any) or injury will be explained during the Pre-Job Meeting.
(PJSM).
• It is recommended to use less hazardous substances to minimize risks to health and the environment.
• Only trained personnel should handle hazardous materials using the precautions to be taken, as
instructed in the SDS.
• Containers used for hazardous materials/chemicals will be disposed of as B3 waste.
• Hazardous chemicals will be disposed of according to the instructions provided on the SDS. Do not litter or
dispose of used/empty chemical containers or use them for storage or other handling purposes as they may
endanger life and the environment.
• Contractor must provide adequate eyewash points, showers and first aid kits with medication at locations
for storage and handling of hazardous chemicals, if deemed necessary. Also, to ensure that it is properly
maintained and inspected regularly.
• Employees must be provided with information regarding work with hazardous substances and the use and
maintenance of applicable PPE. At a minimum, this should be provided as part of the safety induction
training.
• Storage of Hazardous Materials/Materials must pay attention to the following requirements:
• All chemical containers must be labeled.
• The contractor should ensure that adequate cupboards/storage spaces are available for proper
segregation and storage of the various types of hazardous substances/chemicals.
• Incompatible chemicals should be stored separately.
• Areas where hazardous materials are stored must be identified by proper signs indicating the contents of
the storage area and a 'No Smoking' sign.
• Spill kits should be provided where hazardous substances are stored or used.
• The storage area for hazardous substances will be well ventilated and maintained in a hygienic manner.
• Portable fire extinguishers and suitable safety showers should be provided in storage facilities and
employees will be trained to use them effectively in the event of exposure/contact with hazardous
substances/chemicals.
• Contractor will provide spill equipment
• Hand tools and power tools must be checked before use. The Contractor shall ensure that all tools are
kept in a safe condition and used in a safe manner by all personnel.
• Defective equipment will be a red inspection tag (damaged condition) and kept in the Equipment
Warehouse until repaired or replaced.
• Field Supervisors must ensure that workers understand how to use tools properly and care about other
workers.
• The Machine Guard must be inspected to ensure it is operating effectively.
• Covers and safety guards for devices must be kept in place and in an operable condition for all devices.
The wooden handle of the tool must be free of cracks or other defects.
• Color code (inspection tagging) must be installed on the device to determine the condition of the
equipment (Good or Not Good for use).
• Damaged or defective tools must not be used and included in the work area. Re-inspection should be
carried out for these tools after repair.
• Daily checks must be carried out by the tool user to ensure the tool is in good condition and safe.
• Always use the correct tool for each job. Do not use improvised or modified tools.
• Explosion proof devices, or intrinsically safe devices must be used in some hazardous or flammable areas.
• When used, power tools must properly installed distribution box use industry and waterproof plug and
electric socket.
• Using a lot socket (three way socket) no allowed, each tool must be plugged into one socket only
9.2.25 Alat pneumatik / Pneumatic tools
Alat pneumatik termasuk Chipper, Bor, Palu, dan Sanders. Perkakas listrik pneumatik harus diamankan dengan

Pneumatic tools include Chippers, Drills, Hammers, and Sanders. Pneumatic power tools must be secured with
clamps. In addition to mandatory PPE, eye protection (goggles) or face shield (with safety glasses or goggles),
and hearing protection are required for working with pneumatic tools. Pneumatic tools can only be used by
employees who have received training in using pneumatic tools. Employees must follow safe practices for
operating pneumatic tools, including the following:
• Before assembling the system, check all hoses for cuts, breaks and looseness.
• Blow all hoses before installing equipment. If the connector has a rubber seal. Remove before (and
replace after) blowing.
• Protect air ducts or hoses from trucks and pedestrians either by building a runway over the hose or by
hanging the hose from above with the lowest part of the hose at least 7 feet (2 meters) above the ground.
• Tighten the hammer-joint joints with a hammer. Do not tighten by hand. Hoses equipped with special
connections require special tightening techniques or equipment.
• Do not turn on the air valve until the hose connection is secure.
• Do not point the open-air hose at anyone.
• Compressed air should not be used for cleaning purposes unless reduced to less than 30 psi, and then
only with effective chip protection and appropriate personal protective equipment.
• Unless the equipment has a quick change connector (with an internal check valve), shut off the air at
the air supply valve in front of the hose before disconnecting.
The Contractor will ensure that all electrical equipment, such as portable equipment, hand tools and other
portable electrical equipment, meets standard safety requirements.
Contractor will use ELCB (Earth Leakage Circuit Breaker)
Temporary electrical installations including distribution panels, cabling, and switching equipment must comply with
PUIL 2011 standards. All necessary tests will be carried out before use and/or before equipment is returned to
warehouse.
• The LOTO system must be applied before carrying out repairs, adjustments to electrical, pneumatic,
hydraulic, thermal, gravity and radiation equipment.
• Personal Danger Tag tags and padlocks (red color) will be used for equipment that is being repaired by
someone.
• Label (warna kuning) harus dipasang pada peralatan yang rusak atau peralatan yang tidak dapat
