* Fix comment anchor fields; add --parent for threaded replies

The comment command put values in the wrong field, causing the API to
500:
- --selection <text> was sent as anchor_ref (an integer column); it now
  goes in anchor_text.
- --line <n> was sent as a stringified anchor_ref; it now sends the
  integer.

Add --parent <id> so a comment can be posted as a true threaded reply
(sends parent_id). Previously replies could only be posted as top-level
comments, or attempted via --selection (which anchors to text, not a
comment). Combinable with --selection/--line.

Verified against a local server: --selection stores anchor_text,
--line stores integer anchor_ref, --parent threads under the target,
and the combination works. Build passes.

* Add comment-delete command

Consume the new DELETE /docs/:slug/comments/:id endpoint. Requires
--confirm (mirrors the document delete command) and doc-level api-key
auth. Resolves slug from .draftmark.json when only the comment id is
passed. Deleting a comment removes its replies too (server cascade).

Verified against a local server: refuses without --confirm; deletes and
the comment is gone with --confirm.

Fix account key resolution for private doc creation

Account-level keys (acct_...) from global config were shadowed by
doc-level keys (key_...) from .draftmark.json. Now create --private
uses resolveAccountApiKey which checks global config before local,
so dm login --api-key acct_... works correctly.

dm config now shows Account API Key and Doc API Key separately.
