Filter non-unique FK candidates at the VDB query level, not post-hoc

_resolve_via_vdb now passes is_unique: True in the VDB where-clause instead
of fetching every name/description match and discarding non-unique hits
afterward. Same result (a non-unique column still can't serve as an FK's
referenced key), fewer wasted hits to filter through downstream.
