DEFINICIÓN
Síndrome considerado como
una variante o complicación
de la preeclampsia severa y
que está constituido por la
presencia de hemólisis (H),
elevación de enzimas
hepáticas (EL), y
trombocitopenia (LP).

EPIDEMIOLOGÍA
El síndrome HELLP se
presenta en alrededor de
0,1 a 0,8% de todos los
embarazos.
Se presenta en 10 a 20%
de los casos con
preeclampsia
severa/eclampsia.

HEMÓLISIS
La presencia de fragmentos de
globulos rojos (esquistocitos) o
globulos rojos con espiculas
(células Burr) en el frotis de
sangre periférica
refleja el proceso hemolítico y
sugiere fuertemente el
desarrollo de MAHA.

LA TRÍADA DEL SÍNDROME HELLP:
HEMÓLISIS
La destrucción de los eritrocitos de la sangre por
hemólisis ocasiona aumento de la deshidrogenasa láctica
(LDH) y disminución de la concentración de
hemoglobina.
La hemoglobinuria es macroscópicamente reconocibles
en alrededor del 10% de las mujeres.

LA TRÍADA DEL SÍNDROME HELLP:
HEMÓLISIS
La hemoglobina liberada es convertida a bilirrubina no
conjugada en el bazo.
El complejo haptoglobina-hemoglobina se elimina
rápidamente por el hígado.
Concentración de haptoglobina baja (<1 g/L - <0,4 g/L) se
puede utilizar para diagnosticar hemólisis.
El diagnóstico de hemólisis se basa en los niveles de DHL
elevados, niveles elevados de bilirrubina no conjugada pero
el indicador más específico es el nivel bajo o indetectable de
haptoglobina.

LA TRÍADA DEL SÍNDROME HELLP:
ELEVACIÓN DE ENZIMAS HEPÁTICAS
La elevación de las enzimas hepáticas puede reflejar el
proceso hemolítico, así como también una afectación
hepática.
La elevación de los niveles de aspartato aminotransferasa
(AST) y alanino aminotransferasa (ALT) se deben
principalmente al daño hepático.

ELEVACIÓN DE ENZIMAS HEPÁTICAS
La glutatión S-transferasa-a1 (α-GST o GST-a1) puede
proporcionar un indicador más sensible para el daño
agudo en el hígado y permitir el reconocimiento precoz.

Los conteos de plaquetas <100, 000/ul es obligatorio
en el síndrome HELLP (de acuerdo con la definición
de Sibai).
Disminución del conteo de plaquetas en el síndrome
de HELLP se debe a su mayor consumo.

DEXAMETASONA 10 mg EV cada12 HORAS

CORTICOIDES PARA EL SINDROME HELLP
11 ensayos (550 mujeres) compararon
corticosteroides con placebo o ningún
tratamiento.
No hubo diferencias en el riesgo de muerte
materna (CR 0,95; IC95%: 0,28-3,21), muerte
materna o morbilidad materna grave (CR 0,27;
IC95%: 0,03-2,12) ni en la mortalidad
perinatal/infantil (CR 0,64; IC95%: 0,21-1,97).
El único efecto claro fue una mejoría en el
recuento plaquetario (DME 0,67; IC95%: 0,24-
1,10). El efecto fue mayor para las que
comenzaron el tratamiento en el período prenatal
(DME 0,80; IC95%: 0,25-1,35).

CORTICOIDES PARA EL SINDROME HELLP
2 ensayos (76 mujeres) compararon
dexametasona con betametasona.
No hubo pruebas claras de una diferencia entre los
grupos en cuanto a la mortalidad perinatal/infantil
(CR 0,95; IC95%: 0,15-6,17) ni en la morbilidad
perinatal/infantil grave o la muerte (CR 0,64;
IC95%: 0,27-1,48).
En cuanto al recuento plaquetario, la
dexametasona fue superior a la betametasona
(DM 6,02; IC 95%: 1,71-10,33) cuando el
tratamiento se comenzó en el período prenatal
(DM 8,10; IC 95%: 6,23-9,97) y posnatal (DM
3,70; IC 95%: 0,96-6,44).
