Variantes clínicas
Acné inverso
Inflamación crónica de las glándulas apocrinas.
Se da sobre todo en pacientes jóvenes. Afecta a ingles, axilas y periné.
En la fase aguda aparecen nódulos y abscesos que drenan pus, posteriormente se forman
tractos sinusoidales y cicatrices, que perpetúan el proceso.
● En la fase aguda se trata con antibióticos orales, corticoides intralesionales y drenaje de
los abscesos.
● En la fase crónica se emplean retinoides orales y cirugía.
● En los casos refractarios está aprobado el uso de adalimumab (anti-TNF).
En ocasiones se asocia a acné nódulo quístico grave y a fístulas pilonidales.

Síndrome SAPHO (Sinovitis, acné, pustulosis, hiperostosis y osteoartropatía seronegativa)
Está considerado un trastorno poco frecuente.
➔ Desde la adolescencia hasta la edad adulta tardía, con una mediana de edad entre los 30 y 40 años.
Etiología
Las bacterias como Propionibacterium acnes, podrían actuar como un factor
desencadenante.
Cuadro clínico
Abarca un amplio espectro de anomalías caracterizadas por una combinación variable de
manifestaciones osteoarticulares y cutáneas de diversa gravedad.
Afectación dermatológica
➔ Acné intenso
➔ Pustulosis palmoplantar
➔ Psoriasis pustular
Comienzan a menudo 1-2 años antes de los cambios óseos, pero pueden aparecer
simultáneamente.
Variantes clínicas
Síndrome PAPA
Raro, Hereditario, Artritis piógena estéril, pioderma gangrenoso y acné quístico; puede llegar a
producir destrucción articular grave.
Acné neonatorum e infantil
Epidemiología
➔ Más frecuente en varone
➔ Se observa en hasta 20% de los recién nacidos y puede aparecer aún durante las primeras semanas de vida
Etiología
Glándula suprarrenal fetal desproporcionadamente grande. Mayor cantidad de dehidroepiandrostenediona (DHEA)
Cuadro clínico

● Pápulas y pústulas

Aparece entre el tercer y sexto mes y desaparece entre los
tres y cuatro años de edad.
Variantes clínicas
Acné androgénico
Es consecuencia de la producción excesiva de andrógenos por ovarios poliquísticos (síndrome
de Stein-Leventhal) u otras alteraciones endocrinas, como el síndrome adrenogenital o de
Cushing.
Suele estar representado por el
síndrome SAHA:
● Seborrea
● Acné
● Hirsutismo
● Alopecia
El diagnóstico se confirma por mediciones hormonales: testosterona plasmática total y libre,
androstenediona, sulfato de dehidroepiandrosterona (DHEA-S), hormona estimulante del
folículo y prolactina, además de ecografía ovárica
Dermatosis acneiformes
Etiología
Ocurren por inducción externa o por contacto, y reciben el nombre de los productos relacionados:
➔ acné cosmético
➔ acné por pomada
➔ acné por detergente
➔ acné mecánico
➔ acné estival por cremas o fotoprotectores
➔ acné por corticosteroides
Variantes clínicas
Rosácea fulminans o pioderma facial
Es una dermatosis acneiforme inflamatoria crónica, que afecta generalmente a mujeres entre
30 y 50 años.
Etiología Cuadro clínico
La etiología es desconocida. Se ha relacionado con el ácaro Eritema facial persistente con telangiectasias.
Demodex folliculorum, con factores que influyen en la labilidad Posteriormente hay brotes de pápulas y pústulas, sin
vasomotora (dieta, café, té, alcohol, picantes, sol), con comedones, en mejillas, nariz, frente y mentón. En el varón
factores endocrinos y con disbacteriosis intestinal. es frecuente la rinofima (hiperplasia de los tejidos blandos de
la nariz). En ocasiones cursa con blefaritis y conjuntivitis
Edema sólido facial o enfermedad de Morbihan
Etiología
Asociación o complicación del acné o de rosácea, y se relaciona con una alteración del flujo sanguíneo cutáneo y del
drenaje linfático.
Cuadro clínico
Tumefacción sólida y persistente de la parte central de la cara.
Para evaluar gravedad
1. Leve si hay menos de 20 comedones, menos de 15 lesiones inflamatorias o
menos de 30 lesiones en total
2. Moderado, de 20 a 100 comedones, 15 a 50 lesiones inflamatorias o 30 a 125
lesiones en total
3. Grave, si hay más de cinco quistes, más de 100 comedones, más de 50 lesiones
inflamatorias o más de 125 lesiones en total.
Datos histopatológicos
Al microscopio se aprecian los folículos dilatados por una masa córnea, rodeada de un infiltrado
inflamatorio supurativo. La rotura del folículo origina una reacción a cuerpo extraño, y alrededor de las
lesiones quísticas pueden aparecer células gigantes.
Tratamiento
Los objetivos del tratamiento son:
● reducir la producción de sebo
● normalizar la queratinización y revertir la hiperproliferación de queratinocitos
● disminuir la colonización por P. acnes,
● prevenir la formación de comedones, microcomedones y lesiones inflamatorias
○ eliminarlos
Terapia tópica
Estándar para el manejo de acné leve
➔ Lavado con agua y jabón dos veces al día
◆ aplicación de lociones desengrasantes a base de licor de Hoffman
Comedolíticos
● Peróxido de benzoílo al 2.5, 3, 5 y 10%
● Ácido retinoico (tretinoína o vitamina A ácida) al 0.010, 0.025, 0.05 o 0.10%, en
crema, solución o gel
● Isotretinoína al 0.05 o 0.025%
Tratamiento

Acné moderado e inflamatorio
Pueden usarse antibióticos tópicos como:
➔ Fosfato de clindamicina al 1%, o eritromicina al 2%, sola o con acetato de cinc al 1.2% en solución o gel.
➔ Su uso como monoterapia debe evitarse para mitigar el surgimiento de cepas de P. acnes resistentes al
tratamiento.
➔ Éstos pueden ser combinados con peróxido de benzoílo, o retinoides tópicos, o ambos, con lo cual se optimiza su
eficacia.
Acné resistente a terapia tópica
Tetraciclinas vía oral.
➔ Minociclina 50 a 100 mg/día
➔ Doxiciclina 50 a 100 mg/día
◆ Son más eficaces por sus propiedades lipofílicas que les permiten penetrar adecuadamente los
microcomedones
Los antibióticos y antimicrobianos deben administrarse
por periodos no menores de ocho semanas. No se
recomienda combinar antibióticos tópicos y orales porque
no ofrece beneficio adicional y aumenta el riesgo de
resistencia bacteriana
También se puede usar
➔ oxitetraciclina (250 a 500 mg/día)
➔ trimetoprim/sulfametoxazol (80/400 mg dos veces al día)
➔ eritromicina (1 g/día) su uso se limita a pacientes embarazadas y niños menores de ocho años
Se administran por periodos de uno a tres meses
